Preliminary investigations revealed that a fully loaded Shacman truck experienced brake failure while speeding, causing the driver to lose control. The vehicle struck a LASTMA officer who was directing traffic, a pedestrian, and another person about 200 metres away, resulting in fatalities for all three victims.

The truck driver initially fled the scene but was later apprehended by LASTMA officials and handed over to the police. Emergency responders, including LASTMA operatives, the Federal Road Safety Corps, the Nigeria Security and Civil Defence Corps, and police officers, were deployed to the scene. The victims were taken to hospital where they were pronounced dead.

LASTMA General Manager Olalekan Bakare-Oki described the incident as devastating, emphasizing that the officer died in the line of duty. He urged haulage and logistics operators to adhere to stricter safety standards to prevent such tragedies and highlighted the daily risks faced by traffic personnel.
